Please place the following form like you would a normal app. Just use your character's name and some type of dash before placing the word 'vampire'. Like this...
Once this form is filled out in it's entirety, your human application will be moved to the accepted vampires board. Also, your member group will be changed to read "vampire". NOTE: THE ONLY SECTION I WANT COMPLETE SENTENCES IN IS THE 'HOW DID YOU BECOME A VAMPIRE'. THE REST DOES NOT NEED THEM.
